Abstract

The authors present a review of 224 cast use episodes undertaken with 145 children, over a 46 month period, at Children's Rehabilitation Center. During this time, the authors developed criteria which they apply to patient selection along with their considerations regarding treatment, cast check-out, follow-up options, research and the potential for misuse. The features unique to these casts and application procedures are specifically described. Short leg casts are designed to maintain normal alignment of the foot and ankle while prohibiting toe grasp as an habitual or compensatory stabilizing effort. The casts are considered supplemental to the general movement training program, used to adjust the alignment, size and structure of the base of support for standing. With this aid, the therapist can work to improve the organization of weight transfer and movement skills. These casts are used during selected intervals of treatment and management at home and school.
