Abstract
A 68 year old patient was referred after diagnostic transradial coronary angiography with a stuck catheter (Judkins Right 4.5, Cordis, Miami-Lakes, USA) in the proximal radial artery (A). Endovascular removal attempts (stretching, different guidewires) by the cardiologist were unsuccessful. An open surgical approach at the level of the elbow revealed a severely kinked catheter, stuck in the intimal layer of the radial artery (B). After removal, the intimal flap was corrected with a bovine patch. Post-operative duplex showed no diminished flow to the hand. Even though there are several endovascular removal techniques (stretching, balloon, or opposite rotation manoeuvres), surgical extraction is sometimes needed.Image 1
