Abstract

We evaluated the test panels of six transplant centers to determine similarity and comprehensiveness of their donor testing. Viral serology in donor testing answers preoperative questions about human immunodeficiency virus and hepatitis B infectivity of the donor, guides the posttransplant management of the patient, and anticipates future directions of transplantation. In view of the essential information provided by donor testing, we propose a panel that includes tests to detect hepatitis B surface antigen and antibody to HIV-1, HIV-2, hepatitis C virus, hepatitis B core antigen, human T-cell lymphotropic virus type I, cytomegalovirus, Toxoplasma gondii , Epstein-Barr virus, and parvovirus B19. The panel also should include ABO and Rh blood grouping, and rapid plasma reagin and fluorescent treponemal antibody absorption tests for syphilis. The panel will evolve as advances in transplantation occur.
