Abstract
The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) has reported that the number of wrong-site surgeries has increased over the last 6 years. It has also reported that over half of these incidents occurred in either the outpatient surgery department of a hospital or a freestanding outpatient facility.1 In August 1998, the JCAHO published Sentinel Event Alert, examining the problem of wrong-site surgery. At the time, they had 15 cases that had been reported for review. A follow-up issue of Sentinel Event Alert, published in December 2001, included 150 reported cases of wrong-site, wrong-person, or wrong-procedure surgery.
