Abstract

Background: Antimicrobial resistance is an important public health problem which increases hospital stays, associated comorbidities, and hospital costs. This problem exists, not only in hospitals but also in first level clinics, especially in common problems such as urinary tract infections. To better understand this problem it is of paramount importance for every institution to know the characteristics of their local microbiota. This study seeks to describe the sensibility pattern of microorganisms isolated in urocultures of two first level clinics,Clinica Periferica Benito Juarez (CPBJ) and Clinica de Servicios Ampliados en Mexicali (CSAM), in Baja California during 2018. Methods and materials: Data was collected from the CPBJ and CSAM microbiology lab's database of positive uroculture isolates that also had sensibility profiles reported. Data collected included gender, age, isolate obtained and sensibility pattern. Rates and frequencies were calculated. Results: During 2018, 225 positive urocultures were presented in CPBJ and 244 in CSAM. Of the 225 cultures in CPBJ, 41 (18.22%) belonged to males and 184 (81.78%) to females. In CSAM of the 244 cultures, 46 (18.89%) belonged to males and 184 (81.78%) to females. In CPBJ, the most common isolate was of Escherichia coli which was reported in 135 (60%) of cultures followed by Enterococcus fecalis in 14 (5%) of cultures and Klebsiella pneumoniae in 13 (6%) cultures. In CSAM, the microorganisms most frequently isolated was Escherichia coli which was reported in 180 (74%) of cultures, followed by Proteus mirabilis in 21 (9%) cultures and Morganella morganii in 6 (2%) of cultures. Mechanism of resistance were determined to more than two families of antibiotics. Conclusion: Despite being first level clinics, isolates with MDR bacteria were common. High resistances were reported of trimethoprim as well as fluoroquinolones in both clinics
