sperm in the ejaculate after vasectomy. These investigators assessed 347 men (6.4%) who had sperm in the first semen sample among 5,460 men with analyses available after vasectomy. Interestingly, in 1996, 26.8% of the men did not show up for their first analysis among the 7,456 total vasectomies performed, and of 189 patients identified for a second sample by questionnaire who had not already undergone a second vasectomy only 57 (30.2%) actually returned to give a second sample. These investigators found that 56.3% (95% CI 50.7% to 61.7%) or about half of men with sperm on the first sample after vasectomy eventually became azoospermic. They recommend that repeat vasectomy in men without azoospermia be performed only if motile sperm persist for more than 6 months, or if the number of motile sperm remains similar or increases with subsequent semen analyses.
