Abstract
ABSTRACT Fever is one of the commonest symptoms and at times the most problematic entity to diagnose and treat. The monsoons in India bring with them a plethora of diseases viz. malaria, enteric fever, viral haemorrhagic and non-haemorrhagic fevers, and diarrhoeal disorders. We report four cases of scrub typhus from Delhi which presented in the second half of August 2013. All cases presented with fever in the second week of illness and had an eschar, hepatosplenomegaly, azotemia and transaminitis. Gastrointestinal symptoms, thrombocytopenia and hypernatremia were observed in three patients; one patient had thrombocytosis. Leucocytosis, transient rash and ARDS were noted in two cases. Two of the patients succumbed to their illness. It is important to entertain a differential diagnosis of rickettsial diseases in febrile illnesses reporting in Delhi and adjoining areas.
