Abstract
われわれはダーモカメラを用いたオンライン再診を行いグループホームで発生した疥癬の集団感染を終息させた．初診時に往診した入居者は女性6名，年齢94.3 ± 6.5歳の認知症を伴う高齢者であった．全例に搔破行動と搔破痕を認め，3例（50.0%）は略全身に皮疹があった．診断結果は，ダーモスコピーによる疥癬の確定診断3名，好発部位の典型的な皮疹と特徴的な病歴による臨床診断2例，非典型的な皮疹と特徴的な病歴による感染疑い1例で，日本皮膚科学会の疥癬診療ガイドライン（第3版）に準拠して全症例にイベルメクチン200 µg/kgを投与した．ダーモカメラを用いたオンライン再診を週1回合計4回行った．ダーモカメラの接写モードで疥癬虫のダーモスコピー像を，通常モードで丘疹や搔破痕などの皮疹を観察した．オンライン診療は医師と施設の介護士に付き添われた患者との間で行うD to P with Hの形態をとった．治癒に要したイベルメクチンの内服回数は2.7 ± 0.5回で，全患者の内服終了1ヶ月後の初診8週後に感染の終息を確認した．通院が困難な場合など患者環境によってはダーモカメラを用いたオンライン診療は有用であると考えた．
