Editors' Note: Commenting on “Pearls & Oy-sters: Tacrolimus neurotoxicity presenting as an isolated brainstem lesion,” Dr. Chang wonders whether the patient reported had, in fact, posterior reversible encephalopathy syndrome (PRES) rather than a direct neurotoxic effect of tacrolimus. Drs. Saadi and Schmahmann, authors of the article, explain that the patient's blood pressure was within the normal range throughout hospitalization, making PRES unlikely. Critiquing “Safety and immunologic effects of high- vs low-dose cholecalciferol in multiple sclerosis,” Dr. Kantor comments that the Class I evidence of safety/tolerability is incorrect because the trial was not powered to demonstrate such an effect, and because of differences in urine calcium:creatinine ratio, incidence of nausea, and hypercalcemia between the 2 groups.
