Abstract Medication safety in children has additional challenges, besides the ones that are present in adults. The challenges related to medication safety in children have a specific character, too. At every stage, the child is at risk of harm. Prescribing is more complex than in adults, because the dose is related to age and is calculated according to the weight of the child. Weight is often not obtained accurately, so doses can be overprescribed or underprescribed. Children are growing, and therefore dosage changes with age, as their weight changes. The therapeutic index is narrow in children and doses are extrapolated, as there are few clinical trials in children. In addition, the dispensing of medications may be more complex, unless the pharmacy holds the appropriate formulations of the drug. This has an impact on administration, where the smallest error can have a significant impact.
