S11B-04 SESSION 11B: FACE MISCELLANEOUS SIMULTANEOUS FAT INJECTION WITH ADIPOSE-DERIVED STEM CELL AND FOREIGN BODY REMOVAL FOR COMPLICATIONS AFTER FOREIGN BODY INJECTION ON FACE

Abstract

Introduction: Although filler is used worldwide in various medical fields with rare complications, the unsafe materials such as paraffin are used widely in the illegal way. Especially, there are cases with filler or paraffin injected by unauthorized personnel, which results high complication rates and severe functional and aesthetical deformity. It is usually require removal but complete removal is very difficult. Adipose-derived stem cell was used as a form of stromal vascular fraction(SVF) to promote angiogenesis and cell survival. Based on our treatment experience about the filler complications, the authors suggest fat injection as an effective treatment option for severe complications of unauthorized foreign body injection. Methods: The retrospective study was done with 17 patients who were treated for complications after removal of unauthorized foreign body from 2009 to 2014. The mean age of the patient group was 64. All patients were female and got illegal foreign body injection on their faces from various places. They presented with various complications such as infection, irregularity, pain and heating sense. All patients underwent surgical excision of the foreign body with direct or indirect incision. The volume replacement of the foreign body was performed by dermofat graft, fat graft or fat injection. Stem cell, extracted by enzymatic emulsification, was simultaneously applied for 6 patients for volume retention. Results: Complications were mostly minor. Graft resorption happened and two cases went revision in non-stem cell group. Two cases showed temporary nerve paresis, which was spontaneously healed after long-term observation. The Inflammation recurred in one case, which was treated with secondary surgery. Major complications such as skin necrosis, severe infection, facial nerve palsy did not happen. There were no depression, asymmetry or large scar remained, so the patients were satisfied with the results. Conclusion: Based on the authors’ treatment experience, foreign body complications resulted from illegal injection of unsafe materials such as infection, inflammation and skin defect are effectively healed with the dermofat graft, fat graft or fat injection with SVF after surgical excision of the foreign body. The result is satisfactory without depression or over absorption.
