Abstract
今回われわれはS状結腸腹膜垂がループ状にヘルニア門を形成し内ヘルニアと診断しえた1例を経験したのでその臨床的特徴像について報告する．症例は73歳の女性で嘔気・腹痛を主訴に来院され，腸閉塞の診断でイレウス管を挿入したが翌日，症状は改善せず緊急手術となった．腹腔鏡補助下にて手術を施行し約40cmの小腸がS状結腸内側の腹膜垂でできたヘルニア門にループ状に嵌頓していた．腹膜垂が原因となった腸閉塞の本邦報告はわれわれが調べ得た限りでは本症例を含め24例であった．そのうち20例が腹膜垂同士あるいは腹膜垂と腸間膜でできた異常裂孔に小腸が陥入した症例で内ヘルニアの1亜型と判断できるものであった．非常に稀な症例を経験したとともに手術既往がなく原因不明の腸閉塞で左下腹部に最強点の痛みを有する場合には本症を念頭におき対応を考慮すべきである．
