Abstract
大腸癌が壁外性に発育し皮下膿瘍を形成する症例は稀である．今回われわれは通過障害を伴わずに皮下膿瘍を形成したS状結腸癌の症例を経験したので報告する．症例は74歳，女性．以前から左下腹部に腫瘤を自覚し，その直上の皮膚に発赤と膨隆を認め，中心部の皮膚に壊死・潰瘍を認めた．腹部CT検査では骨盤内の腫瘤と連続する皮下膿瘍を認めた．大腸内視鏡検査ではS状結腸下行結腸移行部近傍のS状結腸に全周性の狭窄を認め，内視鏡は通過できなかった．生検でGroup 5，腺癌の結果であった．皮膚膿瘍の切開排膿後68日目にS状結腸切除（D3）および浸潤のみられた腹壁と皮膚の合併切除を施行した．病理組織学的には，腫瘍は漿膜を超えて腹横筋近傍まで浸潤，周囲に膿瘍形成が確認されたが，術後10カ月の現在再発なく経過している．
