Abstract

The clinical performance of the new catheter delivered SelectsecureTM screw in pacing lead 3830 of Medtronic was evaluated in RVOT pacing. Methods In 20 patients (5 female, 15 male) with a mean age of 61.1 years (11–93) with class 1 pacing indication (SSS 9, AV-block 11 and AF 1) the Selectsecure lead was placed in the RVOT. (free wall 11, Septum 9). Measurements were taken with the Medtronic Analyzer. Results Lead placement was possible in all patients. Mean procedure time was 69 min (30–106), fluoroscopy time 10 min (3-15). Mean threshold was 1.09 V (0.3–3.5) initial and 0.46 V (0.3–1.2) at the end of procedure, R-wave was 11.6 mV (3.2–30), Impedance 862 (646–1721). Complications intraoperative dislocations 2, repositioning due to high threshold 2 and low sensing 2, ventricular fibrillation 1. 3 patients showed a dramatic decrease of initial high threshold during the first minutes. 2 postoperative threshold increase > 2 V were seen, but no late dislocations. ![Figure][1] Conclusion RVOT pacing with the Selectsecure pacing system is feasible with low complication rate and shows a high injury potential with high initial thresholds, which tend to decrease dramaticly.
