Abstract

Summary We have presented a case of splenic artery aneurysm which ruptured spontaneously into the lesser sac causing a large hematoma to develop locally. A pseudoaneurysm was formed in the retrogastric space as the partially organized hematoma became walled off by neighboring structures and fibrous tissue. This mass became adherent to the posterior wall of the stomach and finally eroded through it. Subsequent rupture of the pseudoaneurysm at the point where it was adherent to the stomach caused massive hematemesis on at least 2 occasions 5 days apart. Death resulted from intraperitoneal hemorrhage associated with a bleeding diathesis 2 days after partial gastrectomy and splenectomy.
