Abstract

Root Cause Analysis (RCA) in its original Eindhoven model taxonomy (technical, organizational, and human categories) has been used for exploring the initial stages of surgical pathology processing, such as accession and grossing, with the focus on the prevention of specimen misidentification. Compiled in the form of tables, RCA categories can be a framework for questioning procedures and protocols and may reveal suggestions for improvement. There can be differences depending on the specific situation in the laboratory, but the main goal is establishing a chain of safeguards focused on processing details.
