Abstract

Risk stratification is an emerging trend in population health management as a system to identify patients at risk and proactively organise healthcare services. The evolution of health information systems, particularly at regional level, has allowed a data background that enables risk stratification of entire populations. Some regions in Europe are way ahead on developing strategies and programs based on risk stratification and improving chronic and integrated care.Under the European Commission project ASSEHS, we have identified some good practices across Europe as well as proposed an appraisal standard for a comprehensive description and comparison of risk stratification models. We aim to showcase these good practices in a participative workshop where we would share the state of the art of risk stratification tools and implementation practices. Regions like Catalonia, Venetto, Lombardia, Puglia, Euskadi, Valencia and Scotland are among those short-listed. Participant are called to explain their strategy development, risk stratification tool selection, program design and deployment and impact on structures, process and outcomes. Common and specific facilitators and barriers will be identified as lessons learnt from each experience as well as areas that require further development.
