Abstract
Neither of the two seminal coronary artery bypass grafting studies from the Cleveland Clinic that established the superiority of the internal thoracic artery (ITA) grafts over saphenous veins specified which ITA was used to bypass the left anterior descending artery (LAD).1,2 However, the in situ left ITA (LITA) to the LAD was the most popular configuration based on comments made by the authors and their direct feedback. The Society of Thoracic Surgeons guidelines encourage the use of bilateral ITA (BITA) grafting and leaves the options open when it comes to choosing which ITA to use for bypassing the LAD.
