At that time, the many functions of that record become evident. The medical record is documentation of what has occurred previously, not only providing specific history and findings but also recreating the deliberations of previous care givers. It gives life to the progression of the patient's condition and makes it possible for different individuals to provide care with continuity and efficiency. In addition, the very act of adding to the record helps to organize the thinking of the person who writes the note and adds an element of discipline that enhances good care.
