Abstract

Abstract Background: The presacral space, also known as the retrorectal space, is a potential space located anterior to the sacrum and coccyx and posterior to the rectum. This region contains the confluence of the embryologic hindgut, neuroectoderm, and the bony pelvis. As such, multiple different tissue types can give rise to retrorectal lesions. An epidermoid cyst is a kind of developmental cyst seen at this site. Case: This article presents the case of a young female diagnosed incidentally, during a gynecologic examination, with a palpable mass in the retrorectal space. Imaging findings supported the presence of a cystic tumor with a maximum diameter of 2 cm in the retrorectal space. The cyst was removed surgically. Results: Histopathologic examination revealed that the lesion was an epidermoid cyst. One year after surgery, the patient presented in good general condition; she was asymptomatic without clinical and imaging signs of any local–regional recurrence.
