Abstract

Daisy, the ward manager in the children's ward at Roger Park Hospital, was on duty one morning when she heard a scream and rushed to the cleaner's room. She saw one of the patients with a bottle in his hand clearly in considerable pain and distress. It was subsequently discovered that the boy, aged 5 years, had wandered from the main ward and found a bottle of cleaning fluid in a cupboard in the cleaner's store room and had taken a drink from it. At the time of the incident, the ward staff had thought that his parents were supervising him.
