Abstract

A 50-year-old black man complained of pain, bleeding, and swelling in gingiva about 2 months ago. Anamnesis showed systemic arterial hypertension. Extraoral examination revealed enlarged right submandibular lymph nodes. Intraoral examination revealed red-purple ulcerated swellings in the anterior right vestibular and lingual gingiva, vestibular gingiva of teeth 14, 13, 11, and 21, and bilateral hard palate with ulcer on the right side; the right soft palate and tuberosity were also committed. Diagnostic hypothesis was Kaposi sarcoma associated with HIV infection. Incisional biopsy was performed. Histopathologic analysis showed hypercellular fasciculated fibrous connective tissue in submucosa, exhibiting altered blood vessels appearance and vascular proliferation with chronic inflammatory infiltrate and extravascular erythrocytes. Immunohistochemical study was positive for CD31 and human herpesvirus-8. Serologic test for HIV was positive. Patient is on antiretroviral therapy and chemotherapy for sarcoma.
