INTRODUCTION

With the release of An Organisation with a Memory[1] and theoretical work on patient safety,[2] a movement for patient safety began which emphasised a systems approach to Strengths and limitations of this study. ► The study will capture qualitative insight and reflection on patient safety from those involved with and responsible for patient transitions across healthcare and care home settings during the COVID-1­ 9 pandemic. Aims and objectives This study aims to develop a better understanding of incident reporting in relation to transitions in care between hospital and care home, and to codesign a systems-­level response to safety issues for patients transitioning between hospital and care home. To meet these aims, the study has the following seven objectives: 1. To what extent has the COVID-1­9 pandemic influenced how transitions in care are managed safely?
