Abstract

Objective To discuss the effectiveness and security of preestablished precision dual-channel guided by B-ultrasound in one-stage percutaneous nephrolithotomy(PCNL) in the treatment of staghorn calculi. Methods The materials of 23 patients with staghorn calculi treated by preestablished precision dual-channel one-stage PCNL between April 2015 to June 2017 were analyzed retrospectively. Results Totally 23 PCNL cases were all succeeded, with no double kidney surgery in the same time.Dual-channel were established successfully in every single kidney with staghorn calculi. The operation time was 53-157 minutes, average (76.0±16.4)min.The time of dual-channel percutaneous nephrostomy was 7-21 minutes, average (11.0±3.2)min. The hemoglobin at 2 hours after surgery decreased 14.5-42.3 g/L, average(26.7±10.2)g/L. The stone clear rate was 78.2%(18/23) at one-stage. Complications Clavien-Dindo grade I occurred in 5 cases (21.7%), grade II in 3 cases(13.0%). No massive hemorrhage occurred in operation and no cases needed interventional therapy. Conclusions Prior puncture can improve the success rate for establishing the second channel, can improve the stone clear rate.Also, it is safety , the method can be another choice for renal staghorn calculi operation.
