Abstract
A common defined language and structure for unambiguously describing all relevant aspects of radiotherapy incidents is needed in order to share valuable but rare information on incidents within and across institutions. This project, initiated by the AAPM's Working Group on the Prevention of Errors (WGPE), aimed to recommend, through a consensus approach, a structure for reporting and analyzing incidents which would maximize learning opportunities and hence enhance patient safety.
