Abstract

According to this study: Enrollment in a remotely delivered hypertension and cholesterol management program was associated with a reduction in blood pressure and low-density lipoprotein cholesterol levels in a heterogeneous patient population.The navigator-led, pharmacist-supported program could serve as a model for enhancing access, patient engagement, and health outcomes in racial, ethnic, and primary language populations historically underserved in health care.
