Abstract

Objective To investigate the relationship of hepatitis B virus related chronic liver disease with abnormal glucose metabolism and insulin resistance (IR), and to investigate the distribution of IR in chronic liver disease. Methods One hundred patients with chronic HBV infection were collected and divided into two groups according to blood glucose level: elevated blood glucose group (36 cases) and normal blood glucose group (64 cases). The liver function and blood lipid index between the two groups were compared. And HBV genotype of the 100 patients were divided into type B (62 cases) and type C (38 cases), the incidence of IR was compared between patients with type B HBV and patients with type C HBV. According to the classification criteria of viral hepatitis, the subjects were divided into HBeAg-negative chronic hepatitis B group (26 cases), HBeAg-positive chronic hepatitis B group (16 cases) and hepatitis B cirrhosis group (58 cases). The incidence of IR and the IR value were compared among the three groups. Results The serum levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), triacylglycerol (TG) and total cholesterol (TC) in elevated blood glucose group were significantly higher than those in normal blood glucose group, albumin (ALB) level was lower than that in normal glucose group (all P 0.05). The incidence of IR in hepatitis B cirrhosis group was significantly higher than that in HBeAg negative chronic hepatitis B group and HBeAg positive chronic hepatitis B group (P<0.05). HBV DNA was positively correlated with homeostasis model assessment of insulin resistance (P<0.05). Conclusions The levels of serum AST, ALT, TG, TC and ALB in chronic hepatitis patients are closely related to the occurrence and development of abnormal glucose metabolism. The characteristics of IR were different in different genotypes, viral load and infection status of HBV. Key words: Hepatitis B virus; Insulin resistance; Abnormal glucose metabolism
