Abstract

Background: Readiness for hospital discharge is an indicator for patients’ recovery from illness which can be a criterion for hospital discharge. Objective: This interrelationship study aimed to determine factors related to readiness for hospital discharge of surgical patients in Maharaj Nakorn Chiang Mai hospital which were sex, age, hospitalized experience, hospitalized plan, length of hospital stay, quality of discharge teaching and care coordination. Methods: The theoretical framework of this study is derived from theory of transition by Melleis and literature review. Participants were 102 surgical hospitalized patients who were selected as study criteria from Maharaj Nakorn Chiang Mai Hospital, Chiang Mai. The research instruments were the demographic data form, the Quality of Discharge Teaching Scale, the Readiness for Hospital Discharge Scale, and the Care Coordination Scale. The data were analysed using descriptive statistics and point biserial correlation. Results: The results revealed that the Quality of Discharge Teaching and Care Coordination were the significant factors which related to the readiness for hospital discharge significantly. (r = 0.285, p = 0.004 and r = 0.357, p < 0.001) respectively. Conclusion: The study result has implications for nurses in focusing on quality of discharge teaching intervention and the care coordination which will encourage the perception of patients’ readiness for hospital discharge. The discharge planning process should provide individual patients with evidenced-based effective discharge teaching, and the care coordination as well as assess their perceived readiness for hospital discharge.
