Abstract
The treatment of recurrent, refractory post-infarction myocardial ischemia remains an unresolved clinical problem. Aggressive pharmaceutical therapy has limited effectiveness, while percutaneous coronary intervention or coronary artery bypass graft surgery are not possible in these patients, due to various reasons. Currently, alternative methods for refractory angina treatment are being developed for patients with chronic stable angina. The potential of these methods in patients with acute coronary syndrome should be investigated in the future studies.

Summary
Лечение рецидивирующей постинфарктной ишемии миокарда, рефрактерной к терапии, является до конца не решенной проблемой. Под рефрактерной стенокардией (РСт) понимают стенокардию у пациентов, которая сохраняется на фоне оптимальной медикаментозной терапии, и которым проведение реваскуляризации миокарда невозможно или сопряжено с высоким риском. АК могут применяться для устранения продолжающихся или рецидивирующих симптомов ишемии у пациентов с острым коронарным синдромом (ОКС), уже получающих адекватные дозы нитратов и β-АБ, у которых возникает непереносимость лечебных доз одного из этих препаратов, а также для лечения вариантной стенокардии [15,23,24]. Что присоединение ингибиторов IIb/IIIa рецепторов к терапии АСК и гепаринами при ОКС улучшает исходы ЧКВ.
