Abstract
Data on errors and error-likely situations in field tests were gathered from three sources: test documents, interviews with test personnel, and observations of a human factors specialist who participated in several field tests. This report summarizes the data and makes recommendations on how the likelihood of such errors can be reduced by using human factors observers with duties as described in this report.
