In Reply .—Certainly, there are advantages and disadvantages to using autogenous bone grafts to reconstruct anterior skull base defects. Stable support of the brain in cases of large defects is essential. Drs Stevens, Apfelbaum, and Morales provide us with some sound reasons for use of the inner table of the frontal bone flap. If their success rate is high, the technique should be considered for inclusion in the armamentarium of other skull base surgeons. For the small- to moderate-sized defects, however, we will continue to use the pericranial flap alone for reasons described in our article. 1 I congratulate Drs Stevens, Apfelbaum, and Morales for their innovative approach to this difficult clinical problem.
