Slavova et al.1 report susceptibility-weighted MRI (SW-MRI) findings during migraine aura, namely, a hypointense signal revealing a single prominent vein draining the cortical area responsible for the patients' symptoms (the index vein). Despite the major interest of these findings, they require clarification.
