Sir, This interesting study reports that many people with ‘mild untreated’ primary hyperparathyroidism (PHPT) do not progress to require parathyroidectomy during an average of 4.7 years of follow-up.1 The authors also report decreases in serum calcium during follow-up of up to 12 years in this cohort despite concomitant increases in serum parathyroid hormone concentrations.
