Abstract

Despite the advances of new modern multidisciplinary treat­ments of head and neck cancers, the rate of loco­re­gio­nal recurrences remains high. The multimodal systemic treat­ment with cetuximab together with platinum-based che­mo­the­rapy associated with 5-fluorouracil or, more re­cent­ly, with taxanes brings benefits regarding survival, but re­mains a palliative treatment. Re-irradiation as the only treat­ment method or after salvage surgery, in cases where it is feasible, represents a potentially cura­tive option, using tech­niques with intensity-mo­du­lated radiotherapy (IMRT) or even stereotactic body radiosurgery (SBRS). However, the rate of severe toxicity remains significant and the selec­tion of patients must be carefully made. Combining im­mu­no­the­rapy with re-irradiation may provide new perspectives for cancer treatments of the head and neck, taking into ac­count the immunogenic effect of radiation therapy.
