Abstract
Diabetes care is suboptimal in First Nations (FN) populations. The RADAR model is one strategy to improve care: integrated disease registries and electronic medical records for community healthcare workers with centralized care coordination. As part of its evaluation, we qualitatively assessed the organization of diabetes care in participating FN communities across Alberta at baseline.
