Abstract

Handoffs between outgoing teams to oncoming teams can lead to medical errors through miscommunication. This study sought to determine if a handoff bundle reduces adverse events and medical errors among hospitalized patients. The prospective intervention study included 1255 admissions on two inpatient units at a children's hospital. A resident handoff bundle was compared to preintervention data. The bundle included standardized communication, handoff training, a verbal mnemonic, and a new team-based structure plus a computerized handoff tool in one of the hospital units.
