Abstract

The synchronous presence of dual hematological malignancy is an uncommon finding. In the present case, an 82-year-old male, presented with lymph nodal mass which was diagnosed as diffuse large B cell lymphoma (DLBCL) on histopathology and plasma cell myeloma (PCM), was diagnosed with identification of clonal plasma cells in the bone marrow. Interestingly, both clonal plasma cells and lymphoma cells showed lambda light chain restriction. The patient also developed spontaneous tumor lysis syndrome which can be seen in aggressive hematopoietic malignancies. The present case is a rare presentation of synchronous dual hematological malignancies and poses diagnostic and therapeutic challenge.
