Abstract

The T wave in electrocardiographic lead V 1 is normally inverted in children under 12 years old. 1 Although upright T waves in anterior precordial leads are a well-recognized electrocardiographic sign of right ventricular hypertrophy, 1,2 the physiologic basis of this sign has not been elucidated. Children who have undergone balloon pulmonary valvuloplasty provide a unique opportunity to determine whether abnormal upright anterior T waves reflect right ventricular systolic pressure or right ventricular hypertrophy.
