Radiology request forms: reducing inefficiency and improving patient safety

Abstract

Purpose: The interventional radiology request form is a powerful document – with only a short paragraph of clinical information, it can call for invasive investigations. However, referring physicians are not familiar with the contraindications associated with the various procedures. Therefore, the clinical information provided is often insufficient. This risks patient safety and causes delays to patient management.
