Abstract

For complete evaluation of the extent of vascular involvement in cases of cerebrovascular insufficiency, one must visualize both carotid and both vertebral arteries in their entire course. Percutaneous transbrachial arteriography makes this possible. Visualization is mandatory before considering the possibility of surgical removal of obstructive lesions. More than one major vessel is usually involved, and the clinical differentiation between involvement of the carotid artery system and the vertebral-basilar system is frequently not borne out. In 89 patients who submitted to brachial arteriography, 109 lesions were discovered (38% of all visualized vessels). In addition to intraarterial plaques, a number of extraluminal causes for obstruction were also discovered. Extra-arterial fascial bands caused intermittent obstruction of the vertebral artery in some cases, and were easily removed surgically.
