Abstract

The most commonly associated cause of a radial nerve palsy is fracture of the humerus diaphysis. A systematic review of the current literature with regard to prevalence, prognosis and therapeutic option of humeral fracture-associated radial nerve palsy was carried out. The overall prevalence of radial palsy in fractures of the humerus is 12%, correlating with the region of shaft fracture. In 85% of cases recovery can be expected. No significant difference in outcome was noted between a primary surgical treatment and primary wait-and-see and possible secondary surgical strategy. Due to a high spontaneous recovery rate early exploration is frequently avoidable. However, in cases of expected neural lesion early surgical exploration is still recommended.
