However, my concern is that the root causes of these disparities have not been adequately explored. During the past 30 years, the business aspect of medicine has invaded the sacrosanct physician–patient relationship. As a result, family physicians have disappeared, especially in rural areas, and have been replaced by health maintenance organizations and organized group physician practices, which gravitated toward cities and large university medical centers.
