Abstract

An analysis is presented of human errors in nuclear-power-plant systems in conjunction with common-mode failures. The errors evaluated are improper testing, inadequate maintenance strategy, and miscalibration. The methodology used is believed to represent a positive contribution to power plant systems availability by identifying sources of common-mode failure when operational functions are involved.
