Abstract

A prospective cohort study of S-ROM stems in hip revision cases has now reached 20 years. In more simple cases in which a primary stem was used, there were 119 cases with no rerevision for aseptic loosening. In more complex cases, in which a long stem was required, there were 262 cases with 10 (3.8%) rerevision for aseptic loosening. There were 7 cases that required a proximal femoral structural allograft. Three of these cases eventually required rerevision because of graft absorption. The aseptic loosening rate is acceptable and indicates that proximally modular fixation is almost always possible as bone is alive and capable of regeneration.
