Abstract

Introduction.The treatment of diabetic foot ulcers is based on systemic antibiotherapy and appropriate assessment of the need for concomitant surgery. General measures towards a topical cure and unloading the ulcer, and improving metabolic control of the diabetes are also relevant. These infections are considered to be polymicrobial, the protagonists being staphylococcus and streptococcus rather than Gram negative and anaerobic bacilli, which tend to take on a more important role in the advanced and complicated stages. Treatment.Ideally, antibiotic treatment should target identified pathogens. However, because many of these infections constitute a clinical emergency, it is recommended starting the patient on empirical antibiotherapy, which should be selected according to the most usual epidemiology of each case. Either emergency or elective surgical treatment might be necessary to control and/or cure the infection.
