Abstract

A 54-year-old patient presented with increasing bilateral lower limb oedema with a positive Stemmer's sign (a fold of skin could not be lifted up at the base of the second toe) and scrotal involvement. Laboratory tests identified hypoalbuminemia (28g/l), hypogammaglobulinemia (IgA 70mg/dl, IgG 305mg/dl, IgM 29mg/dl), and lymphopenia (650mm−3) affecting all subclasses. After exclusion of renal protein loss and due to lack of clinical evidence for liver and heart disease, protein-losing enteropathy was suspected.
