Abstract

目的 探讨亚低温对幼猪外伤性脑损伤(traumatic brain injury,TBI)的保护作用.方法 将幼猪TBI模型(液压法)随机分为常温对照组(颅温37.5℃~38.5℃;n=10)和亚低温组(34℃±0.2℃;n=10).检测脑脊液中特异性烯醇化酶(NSE)、S-100蛋白(S-100)、磷酸肌酸激酶脑型同工酶(CK-BB)水平变化,监测颅内压的变化以及病理检查脑梗塞体积及脑损伤积分.结果 常温组TBI后48 h颅内压达高峰,峰值(38.7±6.7)mmHg,亚低温组颅内压均维持在较低水平,各时点指标均比常温对照组明显降低(P<0.01);TBI后24 h、48 h、72 h亚低温组脑脊液NSE水平较常温对照组明显降低(P<0.01);亚低温组脑脊液S-100、CK-BB测定值亦明显低于常温对照组各个时点的水平(P<0.05、P<0.001);脑损伤积分及脑梗塞体积亚低温组分别为(10.2±3.0)mm3和(38.4±23.5)mm3,显著少于常温对照组(16.4±4.2)mm3、(85.6±18.2)mm3;P<0.05.结论 颅内压、脑脊液生化指标物NSE、S-100、CK-BB水平,病理分析证实亚低温对幼猪TBI具有较好的脑保护作用。
