Abstract

Background: Disease management strategies have shown promise in reducing heart failure (HF) hospitalizations. Yet, well designed prospective controlled trials are lacking. We have shown that an automated telemonitoring system reporting changes in HF patient's daily symptoms and bathroom scale weight improves patient compliance and hospitalization rates. We have expanded patient numbers, varied demographic and clinical care models, and longer term enrollment. These studies have shown that telemonitoring, initiated by the HF patients and utilizing a telephony interface with a clinical database generating daily exception reports (CHF Tel-Assurance™, Pharos Innovations, Northfield, IL) leads to nearly 90% daily compliance, and reductions in hospitalizations for all cause (50%), and for HF hospitalizations (50–85%), in up to four years of follow-up.
