Abstract

A novel smartphone app-based model of care (TeleClinical Care – TCC) for patients with acute coronary syndrome and heart failure was evaluated in a randomised controlled trial of 164 participants. The trial demonstrated improvements in readmission rates, cardiac rehabilitation (CR) completion, and medication adherence. A process evaluation was designed to identify contextual factors and mechanisms influencing results, as well as identifying methods of improving site and participant recruitment and the delivery of the intervention, for an upcoming trial of over 1,000 patients.
