Abstract

Objective: An ovarian ectopic pregnancy is a rare form of ectopic pregnancy after either natural conception or in vitro fertilization-embryo transfer (IVF/ET). We report a case of ovarian pregnancy after intracytoplasmic sperm injection (ICSI) and embryo transfer. The possible mechanisms of an ovarian pregnancy following treatment with an assisted reproductive technique are discussed. Case Report(s): A 34-year-old woman had been treated with IVF/ET (ICSI) due to primary infertility resulting from male factor. She suffered from severe lower abdominal pain on day 31 after ET. An ectopic pregnancy with internal bleeding was suspected according to the findings of sonography and culdocentesis. Laparoscopy was performed. Because there was no evidence of tubal or pentoneal pregnancy, a wedge resection of the right ruptured ovary was performed under the impression of an ovarian pregnancy. Pathologic findings confirmed the diagnosis of an ovarian ectopic pregnancy. Conclusion(s): The diagnosis of and therapy for an ectopic pregnancy following ovarian hyperstimulation are far more difficult, especially with an ovarian ectopic pregnancy. Laparoscopy has become the ideal tool for the diagnosis and treatment of an ovarian ectopic pregnancy. To decrease patient’s morbidity and mortality, clinicians should consider the possibility of an ovarian ectopic pregnancy after treatment with assisted reproductive techniques.
